    Mom with chronic narcotic use

    I am a dietitian who visited a new mom yesterday. She is currently breastfeeding, although I couldn't get a good nursing hx for her 13 day old infant. Mom has been cleared to nurse, although she is using a number of meds (diazepam, Amphet, Bupropion, Divalproex...then 2 days after deliver went to the ER for celexa, atarax, flexeril, oxycodone). She tested positive prior to delivery for cocaine use (stating 1 time she rubbed on hurting gums). Baby's meconium sreen was positive for cocaine at birth. I have a few questions. 1. is it safe for mom to nurse with all her prescription meds. 2. what is the best way to test the baby (urine, stool?). Please let me know your thoughts.

    This mom is not a good candidate to breastfeed, due to the numerous meds and inconsistency of using these medications. Plus the risk that she may return to cocaine use.

    As for testing, I'd suggest using the urine test in the infant.
